42,621 published California Independent Medical Review decisions, classified by treatment, condition and the standards reviewers named, with a ranked treatment-class release and the test of whether that ranking holds. For appeal-drafting tools, navigation platforms and RCM software that need outcomes to cite. Every figure counts only denials that reached external review, and nothing is licensed until California’s Department of Managed Health Care approves commercial use in writing.

A ranking of treatment classes by past outcome is a forecast, so it is scored out of sample on every release: fitted on 20212023, then measure what those classes actually did in 20242025.

Classes in the top quartile ran 85.7% against 47.9% for the bottom quartile, with no refitting. Only 29 of the 71 shipped classes clear n ≥ 30 in both windows, so the test runs on the larger ones — it is evidence the ranking is stable, not proof that every row is. The figure is recomputed on every release and published whatever it says.

What is not in it. No written decisions. The reviewers’ narratives stay in our build database and are sold to nobody at any price — they are tens of thousands of real medical histories, and a licensed file travels further than a web page. No health plan names either: California removes those before publishing.

If you build an appeal-drafting product

Your letters have nothing solid to cite, and it is not for want of trying — the evidence has not existed in a usable form. It is also not something a drafting tool can publish about itself: a patient-side product quoting its own success rates reads as marketing, and the reviewer on the other side of the letter knows it.

At $6,000/yr the licence costs what about 150 appeal letters cost a patient at one drafting service’s $39.95 price. Nothing is licensed or invoiced until the Department approves commercial use in writing — its own portal says commercial uses must be approved, and a licence we are not yet entitled to grant would be worth nothing to you.

A neutral record is citable precisely because it is also sold to the plans. That is the seat this occupies, and the reason the same price applies to both sides.

We have asked California’s Department of Managed Health Care for written permission to sell products built from its published records, and we will not take payment until it answers.
