Every rate, every class, every finding and every cited guideline is free on this site and always will be. The one paid line is the underlying data, licensed for use inside a product, and it is priced the same for everyone — here is why. Below it are the offerings we removed after testing whether anyone should pay for them, and the reasons.
Nothing is on sale yet. We have asked California’s Department of Managed Health Care for written permission to sell products built from its published records, and we will not take payment until that answer arrives. The prices below are published now so you can read the terms before anyone asks you for money. Why we are waiting.
Every line below is sold to patients, provider appeal teams, health plans, advocates and researchers on identical terms at the price shown. There is no payer rate and no provider rate. A health plan buying the data licence pays what a drafting tool pays.

Nothing is licensed or invoiced until the Department approves commercial use in writing: its own portal says commercial uses must be approved, and a licence we are not yet entitled to grant would be worth nothing to you. Worth knowing. Priced to the question once it is scoped — a range is not a price, and nobody has asked yet. We turn down work that means naming health plans against their reasoning, or that amounts to a league table.

Nothing is held back from the free site
All 42,621 decisions are counted into the classes, review types and guidelines you can already read here. We have not removed a single figure to create a reason to pay. What you buy is the decision-level table, a maintained feed and bespoke analysis — not a paywall over numbers that were free a moment ago.
One thing is missing everywhere, at every price: eleven of our reasoning categories failed their accuracy check, so they appear in no product at all. We would rather leave them out than publish them with a warning label. How we test them.
Not sold, and why

Not sold. California's Independent Medical Review costs the patient nothing and the DMHC Help Center files it for them, so no class figure in this record changes whether a free appeal on a costly denied treatment is worth filing — and a paid document that cannot change the decision is a copy of our own free pages. What the kit would have said is free on the class pages, the findings page and the before-you-appeal page.
Appeal triage
Not sold separately. Every row of the ranked table is already published, at the same figure, on its free treatment page, so the table alone is a sorted copy of the site. And the arithmetic that priced it applied external-review outcomes to a provider's internal appeal queue — a different population, mostly administrative denials across every payer. The ranked release now ships inside the data licence as that licence's readable document.
Market baseline report
Not sold. The figure a plan would pay for — its own overturn rate — is removed from the record by statute, and the enforcement record the report rests on is public. Placing a named plan by action count against every other plan is a ranking of a named company, which this project does not publish or sell. And the law that would have made a market baseline urgent, SB 363's fine when more than half of a plan's reviews in a category are overturned, was held in committee on 13 August 2026.
We are not collecting your email address. Most people find this site in the days after a denial. Asking for your details at that moment so we can sell to you later is not something we are willing to do. When there is something to buy, it will be on this page.
